SERVICE OVERVIEW

The Connecticut Department of Social Services (DSS) has implemented a value-based care initiative for home and community-based service (HCBS) providers. To improve Medicaid member health outcomes, the DSS program aims to create and sustain a value-based fee-for-service delivery model by providing whole-person care through incentive payments to HCBS providers based on defined outcomes. By leveraging Connie’s technology, DSS is enabling HCBS organizations to access their clients’ HCBS program-related data.

The DSS program goals include:

  • decrease avoidable hospitalizations  

  • increase the probability of members going home when discharged from the hospital 

  • increase the number of members meeting their personal goals  

  • decrease health inequities among members served by participating organizations  

DSS HCBS PROGRAM TOOL FEATURES 

  • A filterable Worklist of your clients

  • A detailed Snapshot of your clients’ progress related to program goals 

  • A Dashboard to track how your organization is doing in relation to the first three DSS HCBS program Goals.
