Population Health Analytics Suite

Connie invites qualified vendors to submit proposals for the design, implementation, and support of a Population Health Analytics Suite that leverages Connie’s HIE infrastructure and statewide data assets. The solution will support providers, hospitals, health systems, ACOs, CINs, and payers in improving population health, care coordination, quality performance, health equity, and value‑based care outcomes through advanced analytics, predictive modeling, risk stratification, and actionable reporting.
The selected vendor must demonstrate experience delivering scalable, secure, and user‑friendly population health analytics solutions that integrate with diverse healthcare data sources and provide configurable dashboards, user‑defined reporting, care coordination tools, and predictive insights through the Connie Provider Portal. The solution must support evolving quality measures, interoperability standards, regulatory requirements, and value‑based payment models while maintaining strong privacy, security, and governance practices.
Proposal Submission
Proposals must be submitted electronically by 5:00 PM ET on Friday, August 21, 2026 to submissions@conniect.org.
Late submissions will not be considered.
Questions
All questions regarding this RFP must be submitted in writing no later than 5:00 PM ET on Friday, July 24, 2026. Responses to all questions will be posted publicly to ensure all prospective vendors receive the same information.
